Required Information

Accounts can be opened as individual or joint. During the approval process, your credit report will be pulled in order to help us with an approval decision. You will need the following information for individual or joint accounts:

  • Social Security Number (SSN) or ITIN
  • Date of Birth
  • US Government Issued ID
  • Address, phone numbers
  • Employment Information
  • $10 Cash, Check, Funds Transfer or Money Order (to cover the $5.00 membership fee and $5.00 minimum balance)

Beneficiary

Beneficiary information is required on all deposit accounts. The required information is:

  • Social Security Number (SSN) or ITIN
  • Address, phone numbers
  • Date of Birth
  • Relationship to Primary Owner

Under the USA Patriot Act, all financial institutions are required to obtain, verify and record information that identifies each person associated with an account. When you open an account with JM Associates Federal Credit Union, we will ask you for your name, address, date of birth and other identifying information.

For your protection, your application session will time out automatically if there is no activity for minutes. All information will be lost.
